I am guessing that "well done" and "rare" are different (so you can only have one or the other).

 

There are 3 outcomes for well done and 3 outcomes for rare, so 3 + 3 = 6. 

 

If the combinations could be comined (Eg: well done with ketchup and rare with ketchup), then the answer would be 3*3 = 9 combinations.
